  • For glass repairs visit glass.com/auto or contact a glass repair facility of your choosing, please check the limit of liability for your plan.

How do I submit a claim, or submit for reimbursement?

  • After you have completed your covered service and have a paid receipt to submit, fill in your information in the form below and a claims associate will be in touch with you. If you need some more information about how to use your benefits, please fill out the information below.

I have a payment related question, what do I do?
